MADISON, Tenn. (WZTV) — A West Salem, Ohio woman died Friday after a two-vehicle collision on I-65 south near Madison, Tennessee.
The victim, 76-year-old Phyllas Louise Fone was a back seat passenger in the Dodge Journey minivan, according to Metro Police.
The 56-year-old driver of the Dodge said a white or silver vehicle suddenly tried to switch into his lane of travel and hit the right front tire of his minivan.
The Dodge then became disabled and the white or silver vehicle kept going. The driver of the vehicle behind the Dodge, a Jeep Wrangler, said he saw the Dodge stopped on the interstate and immediately applied his brakes, but was unable to stop before hitting the minivan from behind.
Fone was brought to Skyline Medical Center where she died shortly after arriving. The driver and front seat passenger of the minivan, along with the driver of the Jeep, were transported to Skyline with non-life threatening injuries.
There was no evidence of impairment on the part of either driver.
